About the Explant Procedure
Dr. Anous personalizes every breast implant removal procedure he performs to match his patient’s goals and preferences. He frequently combines explant surgery with mastopexy (breast lift surgery) to correct sagging, and he may use fat transfer to add natural volume to the patient’s breasts during this procedure. Some of the most common breast implant removal techniques are outlined below:
Implant removal is a straightforward procedure wherein the patient’s breast implants are removed through short incisions. (Saline implants can be deflated prior to removal, then taken out via keyhole incisions.) New implants may then be inserted into the capsules of fibrous scar tissue that formed around the patient’s previous implants. If the patient has chosen not to have her implants replaced, Dr. Anous will usually perform a capsulectomy; however, patients with very thin, pliable scar tissue may not require this step.
Implant Removal with Capsulectomy
Implant removal with capsulectomy is a multi-step process where the patient’s implants are removed, then the capsules of internal scar tissue around them are partially or wholly excised. Implant removal with capsulectomy is generally recommended for women who have experienced tissue calcification, capsular contracture, or health concerns related to their implants. In some cases, this technique may also improve aesthetic results (removing thick scar tissue prevents textural abnormalities in the breasts).
En Bloc Breast Implant Removal
En bloc breast implant removal is the explant technique of choice for patients experiencing certain complications, including ruptured silicone implants and BIA-ALCL (a very rare form of lymphoma linked to the use of textured silicone implants). Some patients also request this technique for personal reasons. They are typically concerned about tiny silicone particles entering the body during implant removal.
During en bloc breast implant removal, the patient’s breast implants and scar capsules are removed as single, unbreached units. The capsules are surgically released from surrounding tissue prior to removal, then each capsule is taken out with the implant still inside it.
Though en bloc breast implant removal is appropriate for certain patients, it’s not recommended for everyone. This technique requires lengthy incisions, so it will create more scar tissue than other methods of breast implant removal. It also requires a longer, more complex operation. Some women cannot have en bloc breast implant removal because the scar tissue around their implants has become attached to the ribcage or lungs. For these reasons, en bloc breast implant removal is generally not recommended for women who have no medical reason to pursue this procedure.
Who Makes a Good Candidate for Breast Implant Removal?
You’re probably an ideal candidate for breast implant removal if you meet the following criteria:
-You understand both the risks and benefits of this procedure.
-You have clear goals for explant surgery; i.e., you know whether or not you want to have your implants replaced and you understand the limitations of fat transfer (if applicable to your procedure).
-You’re a healthy weight and don’t have any serious medical conditions.
-You do not currently smoke (or you would be willing to stop smoking for three weeks before and after your surgery).
-You are not currently pregnant or breastfeeding.
Note that breast implant removal is medically necessary for patients experiencing silicone implant leakage. While silicone is non-toxic, leaking implants can cause localized irritation, tissue damage, and infection if left untreated. If imaging tests suggest that one of your implants has ruptured, you should arrange a consultation with a board-certified plastic surgeon as soon as possible.
What to Expect during Recovery after Breast Implant Removal
Breast implant removal recovery is typically brief and uncomplicated. Mild to moderate swelling and bruising are common after this procedure, but discomfort can be managed with prescription pain medication (which we will provide). Patients may be asked to wear surgical drains for about one week to remove excess fluid and minimize swelling.
Most women who have breast implant removal can return to work after 7-10 days, but patients who have a fat transfer along with their explant procedure may need to rest for an additional 2-3 days. All breast implant removal patients must avoid vigorous physical activity for four to six weeks after having surgery.
